Before your appointment ...
(1) Complete your pre-appointment questionnaire(s) (REQUIRED for most appointments)
- To access your questionnaire(s): log in to myCornellHealth and click "Appointments."
- If questionnaires are not completed before your visit, you may be asked to reschedule.
- If a questionnaire you receive does not seem to match your health concern, call us during business hours (607-255-5155) to discuss
CAPS appointments: You will have a separate brief Wellbeing Assessment survey to complete 10 minutes before your appointment. Please see instructions for your appointment type, below.
(2) Upload your insurance information (non SHP members only)
If you have not done so already, please upload a photo of your health insurance card in myCornellHealth:
- If you have your insurance card, take a picture of BOTH SIDES of your card on your cell phone. (If you don't have your card, contact the cardholder and ask them to take a picture of BOTH SIDES of the card and send it to your phone.)
- Log in to myCornellHealth and click on “Insurance Cards,” found on the homepage and in the menu.
- Follow the instructions to upload the picture of your insurance card directly into your health record.
If you are enrolled in Cornell's Student Health Plan (SHP) we will already have your insurance information on file.
Accessing your in-person appointment
Plan to arrive 10 minutes prior to your appointment to complete the check-in process before your appointment begins (CAPS clients must also complete a brief Wellbeing Assessment survey).
When you get to our building, you may check in on the level (floor) where your appointment is scheduled. If you're not sure where to go, check in at the main desk on Level 4.
Late arrival policy: If you arrive more than 5 minutes late for your appointment, you may asked to reschedule.
Accessing your telehealth (Zoom) appointment
Please note that in order to receive telehealth services, you must be on (or near) campus at the time of your appointment.
Plan to check in 10 minutes prior to your appointment to complete the check-in process before your appointment begins (CAPS clients must also complete a brief Wellbeing Assessment survey).
To access your secure Zoom link, log in to myCornellHealth, go to “Appointments,” then click “Appointment Check In.”
CAPS appointments: You must complete a brief Wellbeing Assessment survey before your appointment (this survey is in addition to the pre-appointment questionnaire you may have already filled out):
- After you check in, go to “Surveys” in the menu (listed on the left or by clicking the menu icon) and complete the Wellbeing Assessment survey.
- Return to the “Appointments” page and click “Join Meeting” to start your Zoom session. (The “Join Meeting” link will appear once your provider begins the meeting.)
Late arrival policy: If you check in more than 5 minutes late for your appointment, you may asked to reschedule.
Accessing your phone appointment
At the time of your appointment, your health care provider will call you at the phone number you provided when you scheduled your appointment.
CAPS appointments: Please call us at least 10 minutes before your appointment to allow time to complete a brief Wellbeing Assessment survey before you speak with your provider (this survey is in addition to the pre-appointment questionnaire you may have already filled out):
- Call 607-255-5155 and select #2 to reach a staff member who will check you in for your appointment.
- Once you are checked in, log in to myCornellHealth.
- Go to “Surveys” in the menu (listed on the left or by clicking the menu icon) and complete the Wellbeing Assessment survey.
- After you complete your survey, your provider will call you at the phone number you provided when you scheduled your appointment.
